WHAT IS THE MEDICINE USED FOR?
(Indication) Treatment of upper resp & lower urinary tract infections, early localized Lyme disease & eradication of H. pylori. Prophylaxis of endocarditis.
HOW MUCH AND HOW OFTEN SHOULD YOU USE THIS MEDICINE?
Adult & Adolescent >40 kg body wt 750 mg to 3 g daily in 2-3 divided doses. Tonsillitis 1 g bid. Acute exacerbation of chronic bronchitis 1 g bid. Community-acquired pneumonia 1 g tid. Early Lyme disease (isolated erythema migrans) 500 mg to 1 g tid for 14-21 days. H. pylorieradication Amoxicillin 1 g bid in combination w/ clarithromycin 500 mg bid & omeprazole 20 mg/lansoprazole 30 mg bid for 7-14 days. Prevention of endocarditis 2-3 g 1 hr after surgical procedures. Duration of therapy: Continue therapy for 2-3 days following disappearance of symptoms. β-hemolytic strep infections 6-10 days.
(Contraindications) Hypersensitivity to penicillin. Cross-allergy to β-lactams eg cephalosporins. Co-administration w/ disulfiram.
(Precautions) Severe GI disturbances w/ diarrhea & vomiting; allergic diathesis & asthma; moderate to severe renal impairment; viral infections, acute lymphatic leukemia or infectious mononucleosis. Avoid prolonged use. Maintain adequate fluid intake & urinary output at high doses. Discontinue use if severe persistent diarrhea occurs. Concurrent use w/ digoxin or anticoagulants. May interfere w/ lab test results. Pregnancy & lactation.
(Side Effects) Gastric complaints, nausea, appetite loss, vomiting, flatulence, soft stools, diarrhea, enanthems particularly in the mouth, dry mouth, taste disturbances; cutaneous reactions eg exanthema & morbilliform exanthema, pruritus, urticaria.
